What are the Self-Pay rates?

These are the 6 services frequently provided:

Counseling

@ Initial Session (Intake) - $175.00

@ Counseling Sessions - $90.00

@ PSR Sessions - $36.00 per session

Psychiatry

@ Psychiatric Evaluation - $210.00

@ Follow-Ups - $70.00

Targeted Case Management

@ Sessions - $48.00 per billable hour

Payment Policies

All clients agreed to the financial agreement as part of the consent.
Medicaid Clients - Your policy needs to say "Full Medicaid" in our eligibility system in order to be valid.
If you have a primary commercial plan or Medicare, and Medicaid as secondary, please be aware that you must obtain any EOBs from the insurance company and forward to us, so that we can submit payment claims for the Medicaid plan.
All monies shall be paid at the time of service, and if you elect to pay via credit card, while the therapist is present in the session, before she/he leaves. If you have insurance, we will file a claim with your carrier.
Self-Pay methods of payment include: Cash, Personal Check, Credit Card, and Paypal.
Our billing team will review past-due accounts and will issue statements if you show a balance or if you have a copay or deductible that was not collected.
If you receive a statement from our Billing Department, please remit payment to Lukas Counseling - P. O. Box 784719 Winter Garden FL 34778- Attention Billing Specialist. Please include a copy of the statement with your payment. Please be aware, some statements you may not receive until some time after the service date, that's normal.
